Referring to Mat-Su Health Services is very simple. Please complete the form ‘External Referral Form’ with the appropriate information and complete the attached Release of Information for the patient to sign. Once the form is submitted, it goes directly to our Intake Team who will reach out to the patient and appropriately schedule the patient visit.
