Whether you have insurance, are underinsured or are uninsured, you are welcome to call and schedule an appointment with us. No one will be denied care due to inability to pay.

Accepted Insurances

We accept Medicare, Medicaid, and most other third-party insurances. Please bring proof of insurance to your appointment.

Sliding Discounts for Qualifying Patients

As a federally funded facility, we can offer a discount on our fees. To do this, we need to know your family income and size. We ask that you bring proof of your income to your initial visit to Mat-Su Health Services, Inc. (MHS). This allows us to discount your fees within a range that is based on the U.S. National Poverty Guidelines. You must provide this proof of income on an annual basis.


The income guidelines and acceptable verification for the sliding discount application are listed below. Proof of Income may be faxed to MHS at (907) 352-3373. To obtain more information, please contact our client account representative at (907) 376-2411 .


· Proof of Income


· Last three (3) paystubs


· Recent tax return


· Social Security, disability or pension benefit statements


· Government documentation of income


If you have no family income: A letter of no income from Public Assistance, Unemployment, or another governmental agency is preferred.
