When visiting the clinic for the first time, you will need to bring:

·        Photo ID

·        Insurance proof – Medicaid coupon, Denali KidCare, private insurance or Medicare card

·        If applying for the discounted fee, you will need to bring proof of income

·        Various forms that can be filled out before your appointment
